Keg Carbonation

I have my keg pressure set at 12 psi and 34F, I get a slightly foamy pour but when I put it in a 1 liter bottle and cap it I seem to lose the carbonation in a very short time. l have tried it with the pressure equalized but the carbonation does not seem to stay in the beer. I am using a 3/16 OD line with a picnic tap for the pour. Any ideas?
 
I would guess your bottle is leaking.
 
I tested that. I am flat in about 5 minutes. There is a bit of fix when I open the bottle but the beer has very low carbonation. ??

Is the beer that flat when you pull a glass?

If not, then the bottle has a leak or you need to get a counter-pressure filler which keep the carbonation from coming out while filling
 
I will give the counter pressure another try and use totally new bottles. Thanks for the suggestions.

how much foam are you getting when you fill them? excessive foam can be a result from low carbonation.
 
Putting directly into the bottle I get a lot of foam. Not bad with counter pressure. l have the pressure about 14 PSI. What do you suggest if this is low carlbonation?
 
I would say, if the counter pressure filler doesn't foam up, you are fine. Assume pulling a glass the carbonation is good?
